WHY To reduce undesired variation in treatment of our patients. HOW To augment our slow System II -rational- thinking in order to correct our fast -biased- System I. <image:caption>Assistant Professor in AI for Healthcare Systems. Department of Engineering Systems and Services, Faculty of Technology Policy and Management, Delft University of Technology (Delft, the Netherlands) Personal Drive ‘Why do so many promising applications have yet to be adopted in patients’ clinical care or doctor’s workflow?’ WHY The hype around AI triggers hope; from faster diagnosis, improving prognostics or gaining insights to improve efficient use of resources. However, many AI applications have not yet made it to clinical practice. HOW By applying systems thinking, the behaviour of different stakeholders can be identified, enhancing responsible AI for healthcare systems. WHAT The healthcare AI ecosystem is a complex dynamic system. Moving to safe implementation and adoption requires a socio-technical approach, understanding that both ‘social’ and ‘technical’ elements of a problem are equally important.</image:caption>

Doornberg, Prof G.M.M.J. Kerkhoffs, Prof R.L. Jaarsma, A/Prof Joseph H. Schwab. Personal drive ‘Maybe AI methods do not always lead to better outcomes than traditional methods?’ and ‘Can we develop risk stratification tools for prediction of delirium and mortality in hip fracture patients - one of the most common and costly fall-related injuries - to inform treatment decision-making?’ WHY AI may be just another form of predictive analytics, prevention is better than cure, and surgery may not always be the answer in hip fracture patients. HOW To evaluate the hype (or reality) of AI in Orthopaedic Trauma, and develop and validate clinical decision support tools specific to the hip fracture population. WHAT Most hip fracture patients are treated surgically, associated with postoperative complications and high mortality rates and therefore a common theme of discussion in the Orthopaedic trauma literature to optimize perioperative care for these patients.</image:caption>

Proximal tibial tumors can cause a lot of pain and complete immobilization. I strive to provide patients with dependable treatment options, enabling them to regain a level of functionality that fits their lifestyle. WHY The proximal tibia is a common location for tumors and is often treated with resection and reconstruction of the knee. There are numerous methods of reconstruction, and each is accompanied by a host of unique complications. A comprehensive study comparing the clinical outcomes of the various reconstructive techniques is missing in the current literature. HOW We have set up an international multi-institutional study with the goal of establishing a sample size of at least 3000 patients. This will permit a high-quality analysis of the outcomes of each type of reconstruction. WHAT To assemble a large cohort with a wide variety of data from all over the world and investigate the risk factors associated with implant failure. Doornberg, Prof M.P.J van den Bekerom, E.P. van Haarst. Personal drive “Any fool can know. The point is to understand” (Einstein). Breaking complex problems down to simple problems in order to understand the fundamental mechanics motivates me. On daily basis, I use machine learning in clinical setting to improve patient outcomes. I believe that sharing my experiments and knowledge via studies in my PhD track is vital to move forward in order to make machine learning models in hospitals work optimal. WHY Machine learning models have quite sensitive configuration and work better when tuned in an adequate manner. This requires methodological understanding of the possibilities and create improvements where feasible. HOW To expose fundamental mechanics of machine learning models and experiment on methodological variations in order to possibly improve performance and possibly lead to more clinical use and improved patient outcomes. Buijze, (ASI) Personal drive My personal drive is to implement medical research into my daily practice in order to continue improving surgical care. Both innovation and expanding of treatment options, as well as evaluating the efficacy of existing therapies are essential to keep moving forward. In my thesis I aim to combine both of these strategies in order to improve the treatment of rotator cuff tears, one of the most common conditions in shoulder surgery. WHY There are many unanswered questions surrounding the rotator cuff repair. Both the types of tear which should be operated, as well as the surgical approach to such a repair are heavily debated topics. HOW By collecting and analyzing large amounts of patient data using machine learning, we aim to evaluate the efficacy of different types of repair. This information could subsequently be implemented into a decision tool to help physicians in clinical decision making. <image:caption>PhD candidate at the Department of Orthopaedics and Sports Medicine, Erasmus Medical Center, Rotterdam, The Netherlands. Supervisors: Prof. J. Verhaar, Dr. J. Colaris, Dr. M. Reijman. Personal Drive Make sure that patients receive the best care that fits their personal needs. Despite of many research done and protocols published, there is a large variation in treatment strategy for complex distal radius fractures. Instead of treatment based on surgeons preferences, can we turn this into patient and fracture specific care? WHY Help doctors recognizing - in an early stage - whether a distal radius fracture is unstable and requires surgical fixation or that the fracture will remain stable and can be treated conservatively. HOW Collaborate with multiple teams, gather lots of cases of distal radius fractures, and train a machine learning model that will help us predict fracture instability on trauma and/or post-reduction radiographs. WHAT Make sure that this algorithm is available around the globe. This algorithm will give a personalized risk estimation about the redisplacement risk which can guide doctors and patients during early decision making.</image:caption>

Doornberg MD PhD (UMCG Groningen), Mathieu M.E. Wijffels MD PhD (Erasmus MC), Associate prof. Neal C. Chen MD (MGH Boston) Personal Drive Your hand function is one of the most important functions for quality of life and losing (parts of) this function makes you dependent on other people quickly. Distal radius fractures are one of the most common fractures on the globe involving all ages. Therefore optimizing the treatment and preventing complications is of high impact for many people. WHY To improve outcomes after both operatively and non-operatively treated distal radius fractures. HOW Investigating the etiology of and risk factors for complications in patients with distal radius fracture. WHAT Anticipation on predicted complications by minimizing risk factors for, and early recognition of complications.</image:caption>
